Physical Therapy/Occupational Therapy/Speech Therapy

Coverage for up to 30 visits each of occupational therapy, physical therapy or speech therapy. The Participant must be diagnosed by a Physician and the Physician must determine that the therapy is an appropriate course of treatment, based on Medical Necessity for the Participant. The coverage available under the Base Medical Program for the therapy must be exhausted and proof is required to the Benefits Administrator that no additional benefit is available. Participants must obtain Prior Approval from the Benefits Administrator, or the therapy will not be covered.
