Mobility Devices

A Participant, diagnosed with a permanent physical disability requiring use of a wheelchair is eligible for the  reimbursements described below.. To be eligible for reimbursement, the Participant must provide a Physician’s attestation or letter of medical necessity by a Physician specifying use of a wheelchair is required due to a permanent disability. 

Mobility Scooter:  $1,000 reimbursement of the cost of the scooter; once every three Plan Years. No repairs or accessories will be covered.

Standing Frame: $5,000 reimbursement of the cost of the standing frame; once every three Plan Years. No repairs or accessories will be covered.

Stairlift: One time $3,000 reimbursement for the stairlift. Repairs or accessories are not covered.
